PAEDIATRIC CARDIOLOGY

Paediatric Cardiology deals with heart conditions in babies (including unborn babies), children and adolescents. Structural, functional, and rhythm-related problems of the heart are dealt with by this Department.

Congenital heart defects - heart defect present at birth are the most common in heart conditions in children.

Cardiologists, cardiovascular surgeons, anesthesiologists and other specialists work closely together to care for such patients.

CARDIOTHORACIC SURGERY

Each year, the indiahealthtour represented hospitals' Cardiothoracic Surgery departments care for thousands of patients, young and old, who require virtually every type of heart surgery including heart bypass surgery; elective or emergency surgery for aortic aneurysm, coronary artery disease, arrhythmias (such as atrial fibrillation), heart failure, Marfan syndrome, and other less common conditions; and heart valve surgery for heart disease (such as mitral regurgitation or aortic stenosis). Surgeons work with the newest surgical techniques, equipment and devices.

The mortality rate is less than 1%. The patient on whom the first heart transplant was performed 9 years ago, is still well today.

The depth and breadth of experience in dealing a wide variety of cases allows Surgeons to handle more complex operations with success rates at par with the best centres. The entire gamut of cardiac surgery - from surgery on a 2 day-old infant to pacemaker implantation in a 97 year-old has been successfully undertaken .

Cardiothoracic Surgery offers contemporary techniques including off-pump bypass. Over 98% of the surgeries are "beating heart" which offer patients faster post-operative recovery and extract less of a toll on their systems. Beating-heart surgery allows the patient to forego cardiopulmonary support. The precise area of operation is stilled with high-tech stabilizing devices.

The team regularly performs surgeries like valve repair, replacement, and combined surgical procedures using tissue valves and mechanical valves.

The surgery is supported by a well-equipped third generation Cardiac Catheterisation Lab, Critical Care Units and Intensive Care Units.

PAEDIATRIC CARDIOTHORACIC SURGERY

A wide variety of pediatric cardiac surgical procedures have been done including operations on simple congenital lesions such as atrial septal defect, ventricular septal defect, patent ductus arteriosus, coarctations of aorta and Tetralogy of Fallot corrections and more complex procedures such as Glenn and Fontan procedures and Sennings and arterial switches. Patients with atrial septal defects and patent ductus arteriosus are evaluated and assessed for closure by device thereby avoiding surgery. When surgery is considered for atrial septal defect, the option is given to the patient to have the operation through a thoracotomy thereby avoiding a midline sternotomy.

Indiahealthtour.com represented hospitals have several senior cardiologists who are experienced at handling infants with congenital heart disease. They are backed by a well-equipped catheterization unit and echocardiography department besides a 30 bed CCU and ICU. In support, there are several senior anaesthetists and perfusionists and postoperative care is reinforced by excellent nursing staff.

CARDIOVASCULAR IMAGING

Obtaining and interpreting images of the heart is critical to the successful management of any cardiac disorder. The Cardiovascular imaging physicians are experts in the treatment and long-term management of many types of heart conditions including : -

Imaging specialists utilize the following diagnostic studies to diagnose heart disease, plan optimal care for patients, and help ensure treatment is complete. They also use these diagnostic studies to minimize complications during many types of catheter-based procedures and heart surgery.

Echocardiography, including Doppler and 3-D is used to assess the overall function of the heart, determine the presence of many types of heart disease, follow the progress of valve disease and evaluate the effectiveness of medical or surgical treatment.

Stress echocardiography uses exercise or medication to make the heart work harder than when at rest. This helps us obtain more detailed pictures of the heart and how well or poorly it is functioning.

Transesophageal echocardiography (TEE) is used during surgery of the coronary arteries, valves or aorta. TEE is also done to evaluate whether a patient who has atrial fibrillation is at high risk for stroke. The procedure can show the presence of any blood clots in the heart (which have the potential to travel into the bloodstream and cause a stroke.

Intracardiac echo is valuable during electrophysiology and other catheter-based procedures to assist the cardiologist in visualizing the heart's structures and minimizing complications.

Nuclear imaging studies are used to diagnose various heart conditions. Specialists use Multigated Acquisition Scan (MUGA)m positron emission tomography (PET) and sestamibi exercise stress testing to find out more about the structure and function of the heart.

Coronary computed tomography angiography (CT Angiography) is used as a noninvasive method for detecting blockages in the coronary arteries. 64- slice CT Angio system is used accurately detect any significant narrowing of the major coronary arteries and non-invasively detect "soft plaque," or fatty matter that may lead to future problems if lifestyle changes or medical treatment are not initiated. It is also used to determine the conditions of the stents and bypass surgery grafts of patients who have had angioplasties or CABG.

Heart failure management programme

The heart failure programme is established for the care of people with heart failure, also called congestive heart failure or ventricular dysfunction. The programme provides a single location where cardiologists, cardiac surgeons, and clinicians work together toward the common goal of treating diseases such as cardiomyopathy and ischemic coronary disease and controlling congestive heart failure symptoms, such as shortness of breath, fluid retention, and swelling.

There is a Centre for Advanced Heart & Lung disease which provides comprehensive evaluation and risk stratification of PTST Congenital heart failure. An CPET (Cardiopulmonary exercise test) is available, which is a non-invasive investigation, which allows prognosticating in heart failure.

A pulmonary hypertension clinic is being developed which includes evaluation simultaneously by a Pulminologist, Cardiologist and surgeon with an interest in pulmonary Thromboendarterectomy.
