Understanding The Surgery

The surgeon will then remove the necessary amounts of cartilage and skin required to achieve the right effect. In some cases, the surgeon will trim the cartilage, shaping it into a more desirable form and then pin the cartilage back with permanent sutures to secure the cartilage.

In other instances, the surgeon will not remove any cartilage at all, using stitches to hold the cartilage permanently in place. After sculpting the cartilage to the desired shape, the surgeon will apply sutures to anchor the ear until healing occurs to hold the ear in the desired position.



Before surgery

We will evaluate your medical history and will advise you with regards to cessation of smoking and medications containing aspirin. The ears will be examined, evaluated and photographed. The areas that will be excised or reshaped are marked and digital photographs taken in order to document the amount of improvement after surgery.


The Procedure

It can be performed under local or general anaesthesia. An incision is made on the back of the ear in the crease and the cartilage is exposed. The cartilage is modified (excised or reshaped) depending on the preop plan. The angle between the ear and the head is reduced and permanent stitches are used to retain the ear in the new position. The incision is closed with stitches under the skin and a fluffy cotton dressing is done. Bandage is placed on the ear to help in healing. The procedure takes only a couple of hours.

What Happens During Ear Surgery?

Step 1 – Anesthesia Medications are administered for your comfort during the surgical procedure. The choices include local, intravenous sedation or general anesthesia. Your doctor will recommend the best choice for you.


Step 2 – The incision

Correction of protruding ears uses surgical techniques to create or increase the antihelical fold (just inside the rim of the ear) and to reduce enlarged conchal cartilage (the largest and deepest concavity of the external ear). Incisions for otoplasty are generally made on the back surface of the ear. When incisions are necessary on the front of the ear, they are made within its folds to hide them. Internal, non-removable sutures are used to create and secure the newly shaped cartilage in place.

Step 3 – Closing the incisions

External stitches close the incision. Techniques are individualized, taking care not to distort other structures and to avoid an unnatural “pinned back” appearance.

Step 4 – See the results

Ear surgery offers near immediate results in cases of protruding ears, visible once the dressings that support the new shape of the ear during initial phases of healing are removed. With the ear permanently positioned closer to the head, surgical scars are either hidden behind the ear or well-hidden in the natural creases of the ear.


Risks

Complications such as blood clots and infection are uncommon.

After The Procedure

The ears are covered with a bulky bandage after surgery. Any tenderness and discomfort can easily be controlled with medication. If the surgery is done in a hospital, the child should go home the same day or the next day.

The ear bandages are removed after 2 - 4 days, but the child will need to wear a light head wrap for 2 - 3 weeks to promote healing. The decision of when to return to school and normal activities depends on how fast the child heals.


Aftercare

Wound Care - Incisions may be cleaned with dilute peroxide and coated with bactroban cream as needed.

Activity – Increase activity as tolerated with no heavy exercise for two weeks.

Special Instructions – After dressing is removed, wear a support band at least for three to six weeks. The more it can be worn the first week, the better.
